    2. Thank you for taking the time to fill out the following Lake County Parks 5 Year Master Plan Survey.
      For over 50 years the Lake County Park and Recreation Department has been preserving open spaces, hosting programs, and providing recreational opportunities to Lake County's residents and visitors. While our doors are always open to feedback on the parks and activities we provide, we also like to set aside time every five years to evaluate how we're doing and where we're going. Your opinion as a park user (or future park user) is vitally important to us as we set these goals.
    4. As a thank you for your time you have the chance to win a variety Lake County Prizes throughout the summer as well as a chance to win one big prize at the beginning of the fall! To be entered to win, please fill out your contact information at the end. This information will be kept separate from the rest of the survey, and your responses will remain confidential.
    6. This survey should take about 15-20 minutes to complete (maybe a little longer if you've visited many parks or have additional information you'd like to share). However, in an effort to be responsible with your time, we refrained from asking some more in-depth questions that may be very helpful in our planning process. Check "Yes" here if you don't mind being sent a (completely optional) follow-up survey with more in-depth questions. Again your responses to this survey will be kept confidential. We will pull lists based on responses (such as a list of dog-owners, or runners), but will not know anything more than that.
